Before you can update your name with most Michigan agenciesโincluding the Secretary of State (SOS), voter registration, or professional licensesโyouโll need to update your name with the Social Security Administration (SSA). This ensures all your records match and helps prevent mismatches that could delay other updates.

The form used to change your name with the SSA is called Form SS-5, also known as the โApplication for a Social Security Card.โ

Youโll need to provide original or certified copies of the following:
Note: You do not need to provide proof of your old nameโthe SSA will use your current records for reference.
You can submit your application in person or by mail to your local Social Security office.
There is no fee to change your name.
Once your name change is processed, youโll receive your new Social Security card by mail within 7โ14 business days. Your Social Security number will not change, only your name.
The SSA system must be updated before other agenciesโlike the Michigan SOSโcan verify your new name. Wait at least 48โ72 hours before trying to update your driverโs license or other federal/state records.
