What You’ll Need

Bring the following documents to a Deputy Registrar license agency:

• Certified court order or marriage/divorce certificate

• Your current Ohio driver’s license or state ID

• Proof of Social Security number (must match SSA records)

• Proof of legal presence and Ohio residency (e.g., utility bill, bank statement)
